Credit Application Instructions
1. Complete the Credit Application in its entirety.
2. If the organization is sales tax exempt, complete one of the following:
- If the organization is in the State of Michigan, complete the “Michigan exemption certificate” OR
- If the organization is in the State of Ohio, complete the “Ohio exemption certificate.”
3. Prepare on company letterhead, a written request for the extension of credit.
4. Mail or FAX the credit application, written request, and exemption certificate (if applicable) to:
